A Physical Therapy Treatment Model for Your Needs

When you make the decision to seek physical therapy treatment, you’re doing so because you’re looking for a solution to a specific issue. At Core Physical Therapy, we believe in providing you with a treatment plan that is tailored for your specific condition and to meet your therapy goals. Our physical therapists will work with you in a one-on-one setting, allowing you to feel comfortable with the person providing your treatment and to develop a rapport with your therapist.

What to Expect at Your Appointment

Each session lasts about 45 minutes, but we encourage you to come early to make sure you can get started right away. By law, you are required to show your insurance card and photo ID so we can verify your identity. This measure is for your protection so as to reduce the risk of identity theft.

Prior to your first appointment, you may print and fill out the necessary forms. If you should choose to fill them out in our office, please arrive 15 minutes earlier than your scheduled appointment time. During your initial appointment, your physical therapist will perform a thorough evaluation. We will combine the results of that evaluation with your personal goals and physical abilities to develop your individualized program. During your appointments, your physical therapist will use a hands-on technique to improve joint and muscle function and mobilize your soft tissue. You will also be given exercises that will allow you to continue your therapy at home.

For patients who are referred by a physician, we will contact your physician after your first visit to give them the results of our evaluation and let them know what treatment plan has been developed. Your physician will be given an update on your progress before you return to see them again.

Insurance Billing & Physician Referrals

Here are some important things to remember about insurance billing with Core Physical Therapy. Please contact your insurance company to verify your coverage benefits:

  • You do not need a physician’s referral to seek treatment if you are not required to by your insurance provider.
  • You are responsible for your paying your insurance deductible and co-pay.
  • If you have not met your deductible, the additional amount is billed directly to you.
  • Private pay rates are available for individuals with no health insurance.
  • We accept all major credit cards, including Visa®, MasterCard®, Discover® and American Express®.
  • All Medicare and Medicaid Patients must have a Physician’s referral
